Job Description

Advance your career as a Nurse Practitioner delivering vital home health risk assessments to Medicare Advantage members in Coffee County. This per diem opportunity provides maximum flexibility, allowing you to design your own schedule while making a meaningful impact on patient health. You will work with individuals in Tullahoma, Manchester, Beechgrove, and nearby areas, conducting comprehensive visits that help bridge gaps in care.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-schedulingchoose your preferred days and hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Travel to patient residences throughout Coffee County
  • Review prior and current medical history, medical conditions, and medication usage
  • Offer patient education regarding medical conditions and treatment regimens
  • Identify unmet health needs, making recommendations for follow-up and closing care gaps
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience preferred

In addition to meaningful patient interactions and flexible scheduling, you will receive compensation of $80$90 per completed evaluation, with an added mileage stipend and payment for administrative tasks.
