Deliver vital care and make a positive impact as an experienced Nurse Practitioner performing home health risk assessments. This per diem position offers maximum flexibility with the opportunity to create your own schedule, choosing shifts anywhere between Monday and Sunday and accommodating minimum weekly or bi-weekly hourly commitments.

Responsibilities:
- Complete annual home health risk assessments and post-discharge visits for Medicare Advantage members residing throughout Rockingham County
- Conduct comprehensive reviews of prior and current medical histories, conditions, and medications during 45-minute in-home evaluations
- Educate patients on their health status, medications, and self-care
- Identify care gaps and communicate clear recommendations to patients and healthcare teams
- Administrative responsibilitiesno creation of treatment plans or medication prescribing
Qualifications:
- Current Nurse Practitioner (NP) license in New Hampshire
- Board certification through AANP or ANCC
- Active CPR (BLS) certification
- MSN from an accredited nursing school
- At least 6 months of recent NP experience preferred
- Prior home health or risk assessment experience a plus
