Job Description

Seeking a skilled Nurse Practitioner to deliver essential home health risk assessments across Windham County. This per diem position is ideal for independent, driven professionals passionate about delivering high-quality care in a home setting. The role involves conducting comprehensive evaluations for Medicare Advantage plan members, supporting their health through education and early intervention.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingdesign your own workweek
  • Minimum weekly or bi-weekly hourly commitment required

Core Responsibilities:

  • Complete annual home health risk assessments, typically 45 minutes each, in members homes
  • Review and evaluate patients medical histories, current conditions, and medication regimens
  • Provide informative education on medical issues and medication management
  • Identify care gaps and offer actionable recommendations to enhance health outcomes
  • Visits may include annual health assessments or post-discharge follow-ups
  • No responsibilities for treatment plan development or medication prescribing

Qualifications:

  • Active and unrestricted Nurse Practitioner License in Connecticut
  • Board Certified through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience (preferred)
  • Ability to travel throughout Windham County, including Windham (Willimantic), Killingly, and Mansfield
