Schedule
- Flexible shifts available Monday through Sunday
- Self-scheduling with the freedom to set your hours

Key Responsibilities
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate past/current medical history, diagnoses, and medication use
- Educate patients on managing their health conditions and medication regimens
- Detect gaps in care, documenting recommendations for follow-up or resolution
- Assessments may comprise annual wellness evaluations or post-discharge visits
- No requirement to create treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner (NP) license in West Virginia
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited nursing program
- Current BLS/CPR certification
- Minimum of 6 months recent experience as an NP preferred
