Job Description
Take on a vital and autonomous role as a Nurse Practitioner providing home health risk assessments to Medicare Advantage plan members across Kent County, DE. This per diem opportunity is ideal for clinicians seeking a flexible schedule, the ability to set their own hours, and a rewarding chance to support patients in their homes.
Schedule:
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual or post-discharge home health risk assessments for Medicare Advantage plan members
- Evaluate patient medical history, conditions, and medication usage through in-home visits (approximately 45 minutes each)
- Deliver clear education to patients regarding their health conditions and medication regimens
- Identify any clinical care gaps and recommend appropriate follow-up actions
- Note: No treatment plan creation or prescription writing required
Qualifications:
- Valid Nurse Practitioner license in Delaware
- Board certification through AANP or ANCC
- Current Basic Life Support (CPR/BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience (preferred)
- Excellent communication, organization, and patient education skills
