Job Description
Make a meaningful impact in community healthcare by providing home health risk assessments throughout Columbia County, Arkansas. In this flexible, per diem position, youll visit Medicare Advantage members in their homes, conducting thorough evaluations that support better health outcomes and improve the quality of care for patients residing in areas such as Magnolia, Waldo, and Taylor.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment as preferred
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan participants
- Assess patients' current and historical medical conditions, medication use, and overall health status
- Educate patients regarding their health conditions and medications to promote understanding and compliance
- Identify gaps in care and provide clear recommendations to help close those gaps
- No prescribing of medications or creation of treatment plans is required
Qualifications:
- Current Nurse Practitioner license in Arkansas
- Valid CPR certification (BLS)
- Board certification via AANP or ANCC
- At least 6 months of experience working as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
This opportunity offers compensation of $80$90 per evaluation. Additional stipends are provided for mileage and completion of administrative responsibilities.
