Make a meaningful impact by providing home-based healthcare to Medicare Advantage plan members in the McMinn County area. This per diem opportunity is perfect for skilled Nurse Practitioners seeking autonomy, flexibility, and the chance to apply clinical expertise in a rewarding community setting.
Schedule:
- Flexible shifts available between Monday and Sunday
- Build your own schedule
- Minimum weekly or bi-weekly hourly commitment (based on your availability)
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess prior and current medical history, chronic conditions, and medication use during 45-minute home visits
- Deliver patient education regarding medical conditions and appropriate medication use
- Identify gaps in care or opportunities for improvement and recommend practical next steps
- No treatment planning or medication prescribing requiredfocus on assessment and education
Qualifications:
- Unrestricted Nurse Practitioner license in Tennessee
- Board Certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
