This per diem opportunity is ideal for driven Nurse Practitioners who are passionate about patient-centered care and value schedule flexibility. As a provider of in-home assessments, you'll play a vital role in promoting member well-being without the demands of ongoing patient management.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments within member residences
- Review prior and current medical history, conditions, and medications
- Educate patients on medical conditions and medication management
- Identify care gaps, suggest practical recommendations for improved outcomes
- Focus on assessment and educationno need to create treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner license in Virginia (VA)
- Board Certification through AANP or ANCC
- Active CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum 6 months of recent NP experience preferred
