This per diem opportunity empowers certified Nurse Practitioners to collaborate directly with patients in their homes, supporting comprehensive health outcomes and driving ongoing care improvements.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to fit your availability
• Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Complete annual or post-discharge home health risk assessments for Medicare Advantage members
• Conduct comprehensive reviews of each patient's medical history, current conditions, and medication regimen
• Educate patients about their medical conditions, medications, and preventive care
• Document and report any gaps in care, recommending actionable steps to address them
• Focus solely on assessmentno requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner (NP) license in the state of Virginia
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR certification (BLS)
• 6 months or more of recent NP experience preferred
