Job Description

Experienced nurse practitioners are invited to participate in per diem home health risk assessments in the Amherst County area. This position offers flexible scheduling options, making it ideal for those seeking additional hours or greater control over their workweek. You will conduct comprehensive assessments for Medicare Advantage plan members, typically in their homes, covering locations such as Madison Heights and Amherst. Each assessment averages 45 minutes and can be an annual evaluation or a post-discharge follow-up.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule based on your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Collect and evaluate medical history, current conditions, and medication use
  • Educate members on their health conditions and medications
  • Detect gaps in care and provide clear recommendations
  • No responsibility for treatment planning or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Virginia (VA)
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of recent experience as an NP, preferred
  • Master of Science in Nursing (MSN) from an accredited program

This per diem opportunity provides competitive compensation at $80-$90 per completed evaluation, with a mileage stipend and reimbursement for administrative time.
