Job Description
Take on a rewarding per diem role providing important home health risk assessments for Medicare Advantage members across Warren County. As a Nurse Practitioner specializing in home health, you will play a crucial part in evaluating patient health needswithout the responsibility of treatment planning or prescribing medication. This position offers the flexibility to create your own schedule, with shifts available any day of the week.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours; minimum weekly or bi-weekly hourly commitment required
Responsibilities include:
- Conduct comprehensive home health risk assessmentseach approximately 45 minutesin patient homes throughout Warren County
- Review patient histories, current conditions, and medication usage
- Educate patients about their medical conditions and prescribed medications
- Identify gaps in preventative care or treatment and provide recommendations for follow-up
- Visit types vary, from annual health assessments to post-discharge evaluations
- All visits are focused purely on assessment, documentation, and patient educationthere is no treatment planning or prescribing required
Desired Qualifications:
- Active Nurse Practitioner (NP) license in North Carolina
- Board Certification (AANP or ANCC)
- Current CPR (BLS) certification
- Masters degree in Nursing (MSN) from an accredited institution
- At least 6 months of experience as an NP preferred
