Take on a meaningful role providing essential care as a skilled Nurse Practitioner specializing in home health assessments. This per diem position offers the opportunity to engage with Medicare Advantage plan members across Winnebago County, with visits in areas such as Rockford, Loves Park, and Machesney Park. Your expertise will support patients in the comfort of their homes, focusing on comprehensive risk assessments and empowering members to better manage their health.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Freedom to create your own schedule
• Minimal weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments, each averaging 45 minutes
• Review and evaluate medical histories, current health conditions, and medication usage
• Provide clear education to patients about their health concerns and prescribed medications
• Proactively identify gaps in care and offer practical recommendations for follow-up
• No requirements to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner License in Illinois
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• At least 6 months of recent experience working as an NP is preferred
• Master of Science in Nursing (MSN) degree from an accredited program

This role offers compensation of $80$90 per evaluation, in addition to a mileage stipend and administrative timereflecting the value of your expertise and commitment.
