Job Description
Deliver comprehensive care directly to patients homes as an advanced practice provider, performing risk assessments for Medicare Advantage plan members across Winnebago County. This per diem opportunity offers a high degree of autonomy and flexibility, with the chance to make a direct impact on patient health and wellbeing in a home setting. Home visits are scheduled in communities throughout Rockford, Loves Park, and Machesney Park.
Schedule:
- Flexible shifts available seven days a week
- Set your own schedule
- Minimal weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Perform annual in-home risk assessments (approximately 45 minutes each) for Medicare Advantage members
- Review past and current health status, chronic conditions, and medication history
- Educate patients on disease processes, treatment goals, and medication management
- Identify and document gaps in care; make recommendations for closing these gaps
- No requirement to develop treatment plans or prescribe medication during visits
Qualifications:
- Valid Nurse Practitioner license in Illinois
- Active Board Certification through AANP or ANCC
- Current BLS/CPR certification
- Minimum of six months recent NP experience preferred
- MSN degree from an accredited nursing program
