Job Description

Make a direct impact on patient outcomes by conducting essential home health risk assessments as a Nurse Practitioner. This per diem opportunity offers autonomy, flexibility, and the chance to provide meaningful care to Medicare Advantage plan members throughout Winnebago County, including Rockford, Loves Park, and Machesney Park.

Schedule:

  • Commit to a minimum weekly or bi-weekly number of hours that work for you

Key Responsibilities:

  • Conduct home health risk assessments, averaging 45 minutes per visit
  • Review and assess patients medical histories, current conditions, and medications during in-home visits
  • Provide education to members regarding their health status and medications
  • Identify gaps in healthcare and share actionable recommendations for the members healthcare team
  • Focus on assessmentsno need to create treatment plans or prescribe medications

Qualifications:

  • Active, unrestricted Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC required
  • Current BLS (CPR) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred

You will be compensated $80-$90 per evaluation, with an additional mileage stipend and administrative time compensation.
