Advance your career as a Nurse Practitioner in a per diem role centered on home health risk assessments with a focus on patient-centered care. In this position, youll conduct thorough evaluations of Medicare Advantage plan members within Coahoma County, including areas such as Clarksdale, Jonestown, and Friars Point. Each assessment takes approximately 45 minutes and may be an annual health review or a post-discharge visit. This flexible opportunity enables you to manage your own schedule and make a meaningful impact within the community.

Schedule:

• Flexible shifts available seven days a week (Monday through Sunday)
• Self-scheduling to accommodate your availability
• Minimum required commitment, either weekly or bi-weekly

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, existing medical conditions, and medication usage
• Educate patients regarding their health status and prescribed medications
• Detect care gaps and recommend steps to improve patient outcomes
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Mississippi
• Current CPR (BLS) certification
• Board Certification via AANP or ANCC
• Minimum six months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
