Job Description

A unique opportunity awaits Nurse Practitioners to deliver impactful care through home health risk assessments for Medicare Advantage members. This per diem role focuses on conducting comprehensive health evaluations in patient homes throughout Suffolk County, Massachusetts, including Boston, Revere, and Chelsea. Each assessment visit averages 45 minutes and may include annual health assessments or post-discharge evaluations. Practitioners enjoy a flexible schedulechoose your own shifts, with options available seven days a week and the autonomy to set your weekly or bi-weekly commitment.

Key Responsibilities:

  • Conduct annual home health risk assessments for plan members, ensuring a thorough understanding of their health status
  • Review prior and current medical history, conditions, and medication use
  • Educate patients regarding their health and medications to promote understanding and compliance
  • Identify care gaps and provide recommendations for follow-up or resolution
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner (NP) license in Massachusetts
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current CPR (BLS) certification
  • At least 6 months of recent Nurse Practitioner experience preferred

Compensation for this role is $80-$90 per completed evaluation, with a mileage stipend and administrative time paid in addition.
