As a Nurse Practitioner specializing in home health, you will conduct risk assessments for Medicare Advantage plan members in communities such as Lynn, Lawrence, and Haverhill. Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own work schedule
- Minimum commitment requirement on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patient residences
- Evaluate prior and current medical history, existing medical conditions, and medication usage
- Deliver education to patients regarding their conditions and medication regimens
- Identify and document care gaps, offering actionable recommendations for improvement
- Focus on evaluation and educationno prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- A minimum of six months of current Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
This role provides compensation of $80-$90 per evaluation, along with a mileage stipend and administrative time, rewarding your dedication and expertise.
