- Flexible shifts available Monday through Sunday

Role Overview:
As a Nurse Practitioner, you will visit Medicare Advantage members in their homes throughout Bay County, including Panama City, Lynn Haven, and Panama City Beach. Each assessment averages 45 minutes and may involve annual health assessments or post-discharge visits. Your focus will be on health evaluation and education, without the need to prescribe medications or develop treatment plans.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for members of a Medicare Advantage plan
- Review patient medical histories, current health conditions, and medication usage
- Educate patients about their health conditions and medication regimen
- Identify care gaps and recommend strategies for resolution
- Complete thorough documentation related to each visit
Qualifications:
- Active Nurse Practitioner license in Florida
- MSN degree from an accredited nursing program
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Minimum of 6 months current NP experience preferred
