An exciting opportunity is available for an experienced Nurse Practitioner to provide comprehensive home health risk assessments for Medicare Advantage plan members. Serving individuals in Timberlake, Altavista, Brookneal, and throughout Campbell County, VA, you will play a vital role in promoting overall health and reducing potential risks for patients in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Assess patients previous and current medical and medication histories
- Educate patients on their conditions and prescribed medications
- Identify care gaps and make recommendations to address them
- Perform a variety of home visits, including annual assessments and post-discharge reviews
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner license in Virginia
- BLS (CPR) certification
- Board certification through AANP or ANCC
- At least 6 months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
