Make a measurable impact on patient care by providing essential home health risk assessments. This per diem opportunity lets you support Medicare Advantage members throughout Lincoln Countyincluding Chandler, Stroud, and Praguewhile enjoying a schedule tailored to your availability.
Schedule:
- Flexible shifts available any day Monday through Sunday
- Design your own work schedule
- Meet only a minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform thorough home health risk assessments (approximately 45 minutes each) in client residences
- Review each patients medical history, ongoing conditions, and medication lists
- Deliver education about medical diagnoses and prescribed medications to patients
- Identify care gaps and recommend solutions to ensure optimal health outcomes
- Visits may include annual assessments or post-discharge evaluations
- No need for writing treatment plans or prescribing medications
Preferred Qualifications:
- Valid Nurse Practitioner license issued by the state of Oklahoma
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Completion of an MSN from an accredited nursing program
- At least 6 months of experience practicing as a Nurse Practitioner (preferred)
Enjoy compensation of $80$90 for each completed evaluation. Mileage stipend and administrative support time are provided to ensure youre fully appreciated for your efforts and travel.
