As a Nurse Practitioner specializing in home health, you will play an essential role in completing comprehensive risk assessments for Medicare Advantage members in locations such as Lumberton, Red Springs, and Pembroke. This per diem opportunity offers unmatched schedule flexibility, empowering you to set your own hours and choose the shifts that best fit your lifestyle.
Schedule:
- Flexible shifts available seven days a week (MondaySunday)
- Build your own schedule based on availability
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members
- Review patients prior and current medical history, conditions, and medication usage
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and recommend appropriate interventions for optimal health outcomes
- Focus exclusively on assessmentno prescribing or treatment planning required
- Average assessment duration: approximately 45 minutes per visit
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board certification (AANP or ANCC)
- MSN from an accredited nursing school
- Minimum of 6 months of current NP experience preferred
You can expect compensation ranging from $80$90 per evaluation, in addition to a mileage stipend and administrative time.
