Key Responsibilities:
- Complete annual or post-discharge home health risk assessments with patients, each assessment averaging 45 minutes
- Review and document prior and current medical history, ongoing conditions, and medication use
- Educate patients about their medical conditions and prescribed medications to empower better self-care
- Identify care gaps, providing clear recommendations to support improved health outcomes
- Focus exclusively on assessmentsno treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in GA
- Board certified through either AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of current Nurse Practitioner experience preferred
This position offers competitive compensation, with $80$90 provided per completed evaluation, in addition to a mileage stipend and administrative pay, recognizing your time, expertise, and travel.
