Job Description

This per diem opportunity allows you to utilize your skills as a Nurse Practitioner, focusing on home health risk assessments throughout Scioto County, including areas such as Portsmouth, Wheelersburg, New Boston, and Minford. Youll have autonomy to create your own schedule while contributing to patient wellness in your local community.

Schedule:

  • Flexible shifts available seven days a week
  • Self-scheduling modelchoose the days that work best for you
  • Minimum weekly or bi-weekly hour commitment required

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate current and past medical history, including medication usage
  • Educate patients and families about chronic conditions and proper medication management
  • Detect gaps in care and offer recommendations for follow-up or improvement
  • Each visit averages 45 minutes and may include annual assessments or post-discharge check-ins
  • Prescribing and treatment planning are not required for this role

Required Qualifications:

  • Active Nurse Practitioner License in the state of Ohio
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing school
  • At least 6 months of NP experience preferred
