Experienced nurse practitioners are sought to provide home health risk assessments for Medicare Advantage members across Scioto County, Ohio. This per diem opportunity offers exceptional flexibilitycreate your own schedule with shifts available any day of the week. The role involves traveling to member residences throughout Portsmouth and nearby communities including Wheelersburg, New Boston, and Minford. Each evaluation typically requires approximately 45 minutes; patient visits may center on annual health assessments or follow-up post-discharge assessments.

Schedule:

• Flexible shifts Monday through Sunday
• Self-scheduling
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual and post-discharge risk assessments in patient homes
• Review and evaluate prior/current medical history, conditions, and medication usage
• Deliver patient education on health conditions and prescribed medications
• Proactively identify and address care gaps, recommending next steps as appropriate
• No prescribing of medications or treatment plan development is involved

Qualifications:

• Active nurse practitioner license in Ohio
• Board certification via AANP or ANCC
• Current BLS/CPR certification
• Minimum MSN degree from an accredited nursing program
• At least 6 months nurse practitioner experience is preferred
