Looking for a meaningful per diem role with the flexibility to create your own schedule? This position offers Nurse Practitioners the unique opportunity to conduct home health risk assessments for Medicare Advantage plan members residing in various communities within Washington County. Youll collaborate with patients in the comfort of their homes and make a positive impact through focused, comprehensive evaluations.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review and record prior/current medical history, conditions, and medication information
  • Educate patients about their health and medication regimens
  • Identify gaps in care and suggest actionable recommendations
  • No development of treatment plans or prescription writing required

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board-certified through AANP or ANCC
  • Six months or more clinical experience as an NP strongly preferred
  • MSN degree from an accredited school of nursing

In this per diem role, you will average approximately 45 minutes per home visit, which may include annual assessments or post-discharge follow-ups. Each day offers variety as you serve patients across locations such as Fayetteville, Springdale, and Farmington. Receive compensation of $80$90 per evaluation, including a mileage stipend and administrative time.
