Advance your clinical practice in a flexible per diem opportunity, providing essential home health risk assessments within Winnebago County, Illinois. Working with Medicare Advantage plan members, you will play a crucial role in promoting proactive, high-quality care through on-site evaluations conducted in members residences across communities like Rockford, Loves Park, and Machesney Park.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule for optimal work-life balance
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual health risk assessments, averaging 45 minutes per visit
• Review and evaluate members prior and current medical history, diagnoses, and medications
• Educate individuals on medication management and condition-specific care
• Identify gaps in care and offer clear recommendations for improved health outcomes
• No need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Illinois
• Current CPR certification (BLS required)
• Board Certification through AANP or ANCC
• Minimum of 6 months current NP experience preferred
• MSN degree from an accredited nursing school

You will be compensated at $80$90 per evaluation, along with a mileage stipend and administrative time, reflecting the importance of this valuable care coordination work.
