Job Description

Experienced Nurse Practitioners are sought to provide comprehensive home health risk assessments in Winnebago County, Illinois. This per diem role offers a high level of flexibility and autonomy, ideal for professionals who thrive in independent, patient-focused settings. Assessments are performed on Medicare Advantage plan members and may include annual health evaluations or post-discharge visits. Each visit generally takes approximately 45 minutes; scheduling flexibility allows you to tailor your commitment and locations, which include Rockford, Loves Park, and Machesney Park.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own weekly or bi-weekly schedule
  • Minimum hourly commitments as preferred

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage plan members
  • Collect and review medical histories, current health conditions, and medication lists
  • Educate patients on their medical conditions and medication management
  • Identify care gaps and suggest actionable recommendations
  • No diagnosis, prescribing, or creation of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from an accredited institution
  • At least 6 months of NP experience preferred
