Schedule:

• Flexible shifts available any day between Monday and Sunday
• Design your own work schedule to fit your life
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members (average duration: 45 minutes per visit)
• Evaluate patients current and past medical history, present medical conditions, and review medication history
• Educate patients on their health conditions and prescribed medications
• Detect care gaps and recommend appropriate actions to address them
• No prescribing or forming treatment plans requiredfocus completely on assessment and education

Qualifications:

• Active Nurse Practitioner license in Colorado
• Current CPR certification (BLS)
• Board certified through AANP or ANCC
• Minimum of 6 months recent experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited institution

You will be compensated $80$90 for each completed evaluation, along with a mileage stipend and pay for administrative time.
