The role is based in Weld County, CO, serving communities such as Greeley, Longmont, and Windsor.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Design your own work schedule to fit your life
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members (average duration: 45 minutes per visit)
- Evaluate patients current and past medical history, present medical conditions, and review medication history
- Educate patients on their health conditions and prescribed medications
- Detect care gaps and recommend appropriate actions to address them
- No prescribing or forming treatment plans requiredfocus completely on assessment and education
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months recent experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited institution
You will be compensated $80$90 for each completed evaluation, along with a mileage stipend and pay for administrative time.
