Job Description
Embark on a dynamic per diem opportunity delivering crucial home health risk assessments for Medicare Advantage plan members across Blount County, TNincluding areas such as Maryville, Alcoa, and Friendsville. You'll be based in Louisville, TN, and play a key part in annual or post-discharge visits, focusing on thorough, patient-centered care in a home setting. Each risk assessment averages 45 minutes and aims to support members with valuable education and comprehensive evaluations.
Schedule:
- Flexible shifts available seven days a week (Monday through Sunday)
- Self-managed schedulechoose shifts that fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct in-home risk assessments for Medicare Advantage members
- Review and evaluate medical, prior/current conditions, and medication histories
- Highlight gaps in care and make recommendations to optimize wellness
- Educate patients regarding their health and medications
- No treatment plan development or prescription responsibilities
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR (BLS) certification
- Board Certification (AANP or ANCC)
- Masters degree in Nursing (MSN) from an accredited program
- Six months of recent Nurse Practitioner experience preferred
