Elevate your nursing practice with an impactful per diem opportunity focused on home health risk assessments. In this specialized role, youll visit Medicare Advantage plan members in Blount County, TN, conducting comprehensive evaluations to enhance their health outcomes. Assignments may include annual health assessments or post-discharge visits, with each encounter designed to last approximately 45 minutes. Locations may vary across Maryville, Alcoa, and Friendsville within Blount County, and youll have the flexibility to customize your schedule to suit your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to self-schedule
- Minimum commitment required on a weekly or bi-weekly basis
Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical history, existing conditions, and medication usage
- Provide education and counseling regarding health conditions and medication adherence
- Identify care gaps and recommend solutions, acting as a trusted resource for patients
- No responsibility for creating treatment plans or managing prescriptions
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification from AANP or ANCC
- Current BLS certification (CPR)
- MSN degree from an accredited nursing program
- Minimum of 6 months NP experience preferred
- Strong communication and organizational skills
