Make a meaningful impact in patient care by performing home health risk assessments for Medicare Advantage plan members throughout Santa Fe County, NM. This per diem opportunity is designed for Nurse Practitioners who thrive in flexible environments, value autonomy, and are passionate about preventive care. Schedule:

• Flexible shifts available seven days a week
• Self-scheduling: create your own workdays
• Minimum weekly or bi-weekly hourly commitments

Role Overview:

• Perform comprehensive home health risk assessments, each lasting approximately 45 minutes
• Evaluate medical history, current conditions, and medication usage
• Deliver patient education on health conditions and prescribed medications
• Identify potential gaps in care and make actionable recommendations
• No requirement for prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in NM
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited institution
• At least 6 months of current NP experience (preferred)
• Strong communication, assessment, and documentation skills

This position offers a compensation rate of $80-$90 per evaluation, with additional mileage stipend and paid administrative time. You will serve communities within Santa Fe, Espanola, Edgewood, and nearby areas, contributing to preventative health measures for local Medicare Advantage members.
