Job Description

In this per diem position, you will perform home health risk assessments on Medicare Advantage plan members throughout Weld County, Coloradoincluding Greeley, Longmont, and Windsor. This opportunity is ideal for practitioners who appreciate autonomy and scheduling flexibility, with the ability to create your own schedule and select hours that suit your needs.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Design your own schedule to accommodate your lifestyle
  • Choose your minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Conduct comprehensive, in-home risk assessments, lasting approximately 45 minutes each
  • Evaluate medical history, current conditions, and medication lists
  • Educate members on their health conditions and medications
  • Identify gaps in care and make recommendations for appropriate follow-up
  • Complete annual health assessments or post-discharge visits as assigned
  • No requirement for developing treatment plans or prescribing medications

Required Qualifications:

  • Unrestricted Nurse Practitioner license in the state of Colorado
  • Current Basic Life Support (BLS/CPR) certification
  • Board Certification as a Nurse Practitioner (AANP or ANCC)
  • Completion of a Master of Science in Nursing (MSN) from an accredited program
  • Minimum of six months recent NP experience preferred

Compensation:

  • Compensation per completed evaluation ranges from $80 to $90, including a mileage stipend and compensation for administrative time
