Take on a meaningful per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you will visit Medicare Advantage members in their homes across various areas within Weld County, including Longmont, Greeley, and Windsor. Each visit typically lasts around 45 minutes and may involve conducting annual health assessments or post-discharge check-ins, providing a vital service to help ensure continuity and quality of care for these individuals.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own schedule according to your availability
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review prior and current medical history, ongoing conditions, and medication regimens
- Educate members on their medical conditions and prescription medications
- Identify gaps in care and recommend next steps for resolution
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board Certification through AANP or ANCC
- Current CPR/BLS certification
- MSN from an accredited nursing program
- At least 6 months of Nurse Practitioner experience is preferred
