This home health role empowers Nurse Practitioners to provide essential healthcare assessments for Medicare Advantage members living within Adams County, CO, including the areas of Aurora, Thornton, and Westminster.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own workdays and hours to fit your lifestyle
- Commitment options include minimum weekly or bi-weekly hours
Key Responsibilities:
- Perform comprehensive home health risk assessments, typically lasting 45 minutes, for Medicare Advantage members
- Assess past and current medical history, health conditions, and medication usage
- Educate patients on managing their health conditions and understanding prescribed medications
- Identify care gaps and recommend appropriate follow-up, improving patient continuity of care
- Focus exclusively on assessmentno prescribing medications or developing treatment plans is required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) degree from an accredited institution
- At least six months of Nurse Practitioner experience preferred
