Schedule:

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members residing in various locations, including Grand Junction, Fruita, and Palisade
• Review prior and current medical history, evaluate ongoing health conditions, and assess medication history
• Educate patients about their medical conditions and medication usage
• Identify gaps in care and provide actionable recommendations to address them
• Focus exclusively on assessments; no treatment planning or prescribing medications required

Qualifications:

• Active Nurse Practitioner (NP) License in Colorado
• Board Certification (AANP or ANCC) required
• Master of Science in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• Minimum 6 months of recent Nurse Practitioner experience preferred
