Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage members throughout Cass County, IN. This per diem opportunity offers outstanding scheduling flexibility, allowing you to choose shifts between Monday and Sunday and set your own availability each week or every other week. Assignments may take you to locations such as Logansport, Galveston, Walton, and other areas within the county during your shift.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-determined schedule; opportunity to select preferred days and shifts
• Minimum weekly or bi-weekly commitment required

Key Responsibilities:

• Conduct comprehensive annual risk assessments for home health patients
• Review and document patients prior and current medical history, medical conditions, and current medications
• Educate patients on their medical conditions and medications to promote understanding and self-care
• Identify care gaps and recommend appropriate follow-up actions or referrals
• Facilitate post-discharge assessments as needed
• No prescribing or creation of treatment plans required

Qualifications:

• Active Nurse Practitioner License in Indiana
• Board certification through AANP or ANCC
• MSN from an accredited nursing program
• Current CPR certification (BLS required)
• At least six months of recent experience as a Nurse Practitioner in any setting is preferred
