Deliver high-quality care directly to patients in their homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity places you at the forefront of preventive health, serving Medicare Advantage members throughout communities within Perry County. Candidates will complete comprehensive assessments, offering support and recommendations that make a meaningful impact without the responsibility of ongoing treatment plans or prescriptions.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment ensures adaptability

Key Responsibilities:

• Perform annual risk assessments in patient homes, each lasting approximately 45 minutes
• Evaluate medical history, existing conditions, and medication regimens
• Educate patients about their diagnoses and prescriptions
• Pinpoint care gaps and recommend actions to address them
• All visits conducted as single encountersno follow-up treatments or prescription responsibilities

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification via AANP or ANCC
• Valid CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
