Deliver comprehensive healthcare while enjoying flexible scheduling as a Nurse Practitioner providing home health risk assessments. This per diem opportunity is ideal for professionals committed to improving patient outcomes through personalized, in-home evaluations. As a key member of the home health team, youll work with Medicare Advantage health plan members throughout Spotsylvania County, conducting thorough assessments in locations such as Spotsylvania Courthouse, Lake Wilderness, and surrounding neighborhoods.

• Flexible shifts available Monday through Sunday
• Self-schedulingcreate a workweek that fits your lifestyle
• Minimum hourly commitment required weekly or bi-weekly

Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and document medical histories, current conditions, and medication usage
• Educate patients on their diagnoses and medication regimens
• Identify care gaps and provide actionable recommendations to support patient health
• Focus solely on assessmentsno prescribing medication or creating treatment plans

Qualifications:

• Active Nurse Practitioner license within Virginia
• Board Certification with AANP or ANCC required
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience is preferred

You will receive compensation of $80-$90 per completed evaluation, along with a mileage stipend and consideration for administrative time.
