Job Description

As a Nurse Practitioner specializing in home health, you'll have the rewarding opportunity to independently deliver care to Medicare Advantage members throughout Niagara County. This per diem position is ideal for those who value autonomy and flexibility, as you'll conduct home-based risk assessments that support proactive, patient-centered care.

Schedule & Flexibility

  • Self-schedule your shiftschoose days between Monday and Sunday
  • Minimum weekly or bi-weekly hourly commitment ensures work-life balance

Key Responsibilities

  • Perform comprehensive annual health risk assessments in patients homes
  • Review prior and current medical and medication histories
  • Educate patients on their health conditions and prescribed medications
  • Detect care gaps and provide recommendations to support optimal health outcomes
  • No need to create ongoing treatment plans or prescribe medications

Qualifications

  • Active Nurse Practitioner License in New York State
  • CPR certification (BLS) current and valid
  • Board Certification through AANP or ANCC
  • A minimum of six months recent experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution
