Job Description
Make a difference in patient care by conducting home health risk assessments for Medicare Advantage plan members throughout Niagara County, NY. This per diem position offers the flexibility to create your own schedule, with shifts available between Monday and Sunday. Assignments may take place in Lockport, North Tonawanda, Niagara Falls, and other nearby areas, based on the needs of the community.
Key Responsibilities:
- Conduct comprehensive annual health risk assessments in patient homes, focusing on Medicare Advantage members
- Review and evaluate prior and current medical history, existing conditions, and medication use
- Educate patients regarding their medical conditions and medications to promote understanding and adherence
- Identify gaps in care, providing actionable recommendations for follow-up
- No responsibility for creating treatment plans or prescribing medications

- Flexible shifts available Monday through Sunday

- Commitment can be set on a weekly or bi-weekly basis
Qualifications:
- Active Nurse Practitioner License in New York
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of 6 months nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation includes $80-$90 per evaluation, with mileage stipend and compensation for administrative time.
