Job Description

Advance your career as a Nurse Practitioner specializing in home health by conducting risk assessments for Medicare Advantage members throughout Putnam County, TN. This per diem opportunity offers a flexible schedule, empowering you to create your own shifts that best fit your lifestyle, with availability between Monday and Sunday and minimum weekly or bi-weekly hourly commitments.

Primary Responsibilities:

  • Perform comprehensive annual home health risk assessments for a Medicare Advantage health plan
  • Evaluate and document prior and current medical history, ongoing medical conditions, and medication usage
  • Educate patients about their medical conditions and medication regimens
  • Identify gaps in care and provide proactive recommendations for improvement
  • No requirement to create treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS)
  • National board certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months experience as a Nurse Practitioner is preferred
