Job Description

Deliver valuable care to individuals in a home health setting while enjoying a flexible per diem schedule. Nurse Practitioners are needed to perform risk assessments for Medicare Advantage plan members throughout Overton County. These assessments cover annual health evaluations and post-discharge visits, typically lasting about 45 minutes, and may be scheduled in various local communities throughout the county.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

• Complete comprehensive, in-home risk assessments for Medicare Advantage health plan members
• Review patients prior medical history, current health status, and medication records
• Educate patients on diagnoses, medications, and overall health management
• Spot care gaps and suggest potential strategies for resolution; treatment planning and medication prescription are not required

Qualifications:

• Active Nurse Practitioner License in Tennessee
• Current CPR certification (BLS)
• Board certified via AANP or ANCC
• MSN degree earned from an accredited nursing school
• Minimum of 6 months of current NP experience is preferred

Youll receive $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
