Job Description
Deliver valuable care to individuals in a home health setting while enjoying a flexible per diem schedule. Nurse Practitioners are needed to perform risk assessments for Medicare Advantage plan members throughout Overton County. These assessments cover annual health evaluations and post-discharge visits, typically lasting about 45 minutes, and may be scheduled in various local communities throughout the county.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule for optimal work-life balance
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Complete comprehensive, in-home risk assessments for Medicare Advantage health plan members
- Review patients prior medical history, current health status, and medication records
- Educate patients on diagnoses, medications, and overall health management
- Spot care gaps and suggest potential strategies for resolution; treatment planning and medication prescription are not required
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current CPR certification (BLS)
- Board certified via AANP or ANCC
- MSN degree earned from an accredited nursing school
- Minimum of 6 months of current NP experience is preferred
