Experienced Nurse Practitioners are sought to provide home health risk assessments for Medicare Advantage members across the Syracuse area and Onondaga County, NY, including communities such as Salina, Onondaga, and Clay. This per diem opportunity offers the flexibility to create your own schedule with shifts available any day of the week. Each patient encounter is approximately 45 minutes and may involve annual health assessments or post-discharge visits, supporting proactive and comprehensive patient care in the community.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling to accommodate your availability
- Minimum commitments on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct in-home health risk assessments for Medicare Advantage plan members
- Review and document detailed prior and current medical and medication histories
- Assess ongoing health conditions and educate patients on diagnoses and medications
- Identify care gaps and provide actionable recommendations to support quality outcomes
- Focus exclusively on assessments; no prescribing or treatment plan development required
Desired Qualifications:
- Valid Nurse Practitioner license in New York State
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience preferred
