Job Description

Make a meaningful impact on the health and wellness of Medicare Advantage plan members across Onondaga County, NY through flexible, rewarding home health risk assessments. Nurse Practitioners in this per diem role will visit patients in their homes, conducting thorough 45-minute assessments that contribute directly to improved patient outcomes without the responsibility of prescribing or treatment planning.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own schedule to accommodate personal needs
  • Minimum weekly or bi-weekly commitment required

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Evaluate each patients medical and medication history, current conditions, and overall health status
  • Educate members about chronic conditions, medication regimens, and healthy living practices
  • Identify care gaps and offer actionable recommendations to support continuity and quality of care
  • Visit various locations within Onondaga County, NYincluding Syracuse, Salina, Onondaga, and Clayaccording to shift assignments
  • Focus solely on assessment and education, with no requirement for prescribing medications or developing treatment plans

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • MSN from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current and valid BLS/CPR certification
  • At least 6 months of current NP experience preferred
