Make a meaningful difference by providing high-quality patient care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem role allows you the flexibility to work around your other commitments while supporting communities throughout Warren County, including Norlina, Warrenton, and Macon.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create your own work calendar
• Choose your preferred minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
• Assess and document prior and current medical history, relevant conditions, and medication usage
• Educate patients on conditions and best practices for medication and self-care
• Identify gaps in care and offer recommendations to address those needs
• Visits last approximately 45 minutes and may include annual assessments or post-discharge visits
• No need to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited school of nursing
• Minimum six months of recent NP experience preferred
