Deliver high-quality care directly to patients homes by performing comprehensive health assessments as a Nurse Practitioner specializing in home health. The role focuses on evaluating members of a Medicare Advantage health plan in Warren County, NC, with visits potentially including Norlina, Warrenton, and Macon. This per diem opportunity offers ultimate flexibilityyou set your own schedule with shifts available any day of the week, Monday through Sunday.

Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess patients current and past medical conditions and review medication history
- Deliver clear education to patients regarding diagnoses and medication
- Identify gaps in care, providing actionable recommendations for follow-up
- Focus solely on assessment; no treatment plan development or prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
