A unique opportunity is available for Nurse Practitioners to deliver high-quality care through home health risk assessments within Halifax County, NC. This per diem position offers the flexibility to create your own schedule, with shifts available any day of the week. Practitioners will conduct risk assessments in patients' homes, with visits averaging 45 minutes. Each appointment may involve a comprehensive annual health review or a post-discharge evaluation for Medicare Advantage plan members. Responsibilities may span across Littleton, Scotland Neck, Weldon, and other communities in Halifax County.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual health risk assessments for Medicare Advantage members during home visits
- Review and document prior and current medical history, chronic conditions, and current medications
- Offer patient education regarding health conditions and medication regimens
- Identify care gaps and make recommendations aimed at improving patient outcomes
- There is no requirement to develop treatment plans or prescribe medications as part of these visits
Qualifications:
- Active and unencumbered Nurse Practitioner license in North Carolina
- Board Certified through AANP or ANCC
- Current Basic Life Support (BLS) certification
- Master of Science in Nursing (MSN) from an accredited nursing program
- At least 6 months of recent nurse practitioner experience (preferred)
