Job Description

Expand your expertise in community-based care with this rewarding per diem opportunity in the home health setting. As a Nurse Practitioner specializing in home health assessments, you will conduct in-person risk evaluations for Medicare Advantage plan members residing across Halifax County, NCincluding Littleton, Scotland Neck, Weldon, and Roanoke Rapids. Each home visit lasts approximately 45 minutes and may involve an annual health check or a post-discharge review, giving you the chance to make a tangible difference in patient health outcomes.

  • Commit to weekly or bi-weekly minimum hourly requirements for steady supplemental income

Key Responsibilities:

  • Complete comprehensive in-home risk assessments for assigned Medicare Advantage members
  • Review patients medical histories, chronic conditions, medication regimens, and recent health events
  • Deliver clear patient education on chronic conditions and prescribed medications
  • Identify and document care gaps, making recommendations to close those gaps
  • No ongoing treatment planning or prescribing responsibilities required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Valid CPR certification (BLS)
  • Board-certified through AANP or ANCC
  • At least 6 months of current NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
