Job Description

Experienced nurse practitioners are sought to deliver essential home health risk assessments to Medicare Advantage members across Warren County, NC. In this flexible, per diem position, you will engage with patients in their own homes, completing comprehensive assessments that help bridge care gaps and promote wellness throughout the community. Each evaluation takes approximately 45 minutes and may include conducting annual health assessments or post-discharge visits. You will visit locations within Warren County such as Norlina, Warrenton, and Macon, with shift assignments varying based on area need.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Craft your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate current and prior medical history, conditions, and medications
  • Educate patients on their health status and medication regimen
  • Identify care gaps and make actionable recommendations to enhance patient outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current BLS/CPR certification
  • National board certification (AANP or ANCC)
  • Minimum of 6 months recent experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Those in this role receive compensation of $80-$90 per evaluation, with an added mileage stipend and paid administrative time.
