Advance your nurse practitioner career in a dynamic per diem role, delivering in-home health risk assessments to Medicare Advantage members throughout Adams County, including areas such as Aurora, Thornton, and Westminster. This flexible position empowers you to shape your own schedulechoose the days that work best for you between Monday and Sunday, with the freedom to determine your weekly or bi-weekly hourly commitment.

Key Responsibilities:
- Conduct thorough annual home health risk assessments in patient homes
- Evaluate and review both previous and current medical history, medical conditions, and medication usage
- Provide clear education to patients regarding their health conditions and medication regimens
- Recognize gaps in care and offer actionable recommendations for improvement
- Note: No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
