Job Description

Schedule:

Key Responsibilities:

  • Conduct thorough annual home health risk assessments in patient homes
  • Evaluate and review both previous and current medical history, medical conditions, and medication usage
  • Provide clear education to patients regarding their health conditions and medication regimens
  • Recognize gaps in care and offer actionable recommendations for improvement
  • Note: No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
