Enhance patient care in the community by conducting important home health risk assessments for Medicare Advantage plan members. This per diem Nurse Practitioner opportunity is ideal for professionals seeking flexibility, independence, and meaningful patient interactions across various locations within Greene County, including Linton, Bloomfield, and Jasonville.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment that matches your availability
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members in the patients residence
- Review and evaluate past and current medical history, including existing conditions and medication use
- Educate patients on their medical conditions and medications, supporting their understanding and involvement in care
- Identify any gaps in care and provide recommendations to improve health outcomes
- No prescription writing or formulation of ongoing treatment plans required
Qualifications:
- Active Nurse Practitioner license in the state of Indiana
- Board Certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- MSN degree from an accredited nursing program
- A minimum of six months of Nurse Practitioner experience preferred
This position offers compensation of $80-$90 per evaluation, in addition to a mileage stipend for travel and administrative time, giving you the freedom to maximize your earning potential.
