Flexible Schedule:

• Shifts available any day, Monday through Sunday

• Minimum weekly or bi-weekly hour commitment required

Key Responsibilities:

• Conduct home health risk assessments for Medicare Advantage plan members located in Rockland, Camden, and nearby areas in Knox County
• Evaluate prior and current medical history, including conditions and medication use
• Educate patients about their conditions and prescribed medications
• Identify any gaps in care and offer recommendations for improvement
• All visits are risk assessmentsno requirement for treatment planning or prescribing

Required Qualifications:

• Active Nurse Practitioner license in Maine
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing school
• At least 6 months of Nurse Practitioner experience preferred
