Make a meaningful impact by providing essential home health risk assessments to Medicare Advantage members throughout Lincoln County. In this per diem opportunity, you will utilize your advanced clinical skills as a Nurse Practitioner to support comprehensive, member-focused health assessments within the comfort of patients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own work schedule
- Minimum weekly or bi-weekly hourly commitment to suit your preferences
Key Responsibilities:
- Conduct thorough home health risk assessments, with each visit averaging approximately 45 minutes
- Perform evaluations of medical history, current health conditions, and medication use
- Deliver education to participants about their conditions and medication regimens
- Identify gaps in care and provide actionable recommendations to address them
- Visits may include annual health assessments or post-discharge evaluations
- No need to create treatment plans or prescribe medications
- Some travel within Lincoln County (including the Lincolnton area, Iron Station, Lowesville, and Westport) will be required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Certification from AANP or ANCC
- Graduation from an accredited school of nursing with MSN degree
- At least 6 months experience as a Nurse Practitioner preferred
