Deliver comprehensive care by conducting home health risk assessments for Medicare Advantage plan members throughout Benton County, MO. In this per diem Nurse Practitioner role, youll provide vital assessments to individuals in their homes, ranging from annual evaluations to post-discharge visits. The flexible, autonomous nature of the position allows you to create a personalized schedule, with shifts available any day of the week. You may be assigned to visit members residing in diverse areas such as Warsaw, Lincoln, Cole Camp, and White Branch.

Schedule:

• Flexible shifts offered Monday through Sunday
• Self-scheduling for optimal work-life balance
• Set your own minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Review each patients current and prior medical history, chronic conditions, and medications
• Educate patients on managing health issues and proper medication usage
• Identify gaps in care and recommend resolutions to improve health outcomes
• No ongoing treatment planning or medication prescribing required

Qualifications:

• Current Nurse Practitioner license in Missouri
• Active CPR certification (BLS)
• Board certified through AANP or ANCC
• MSN degree from an accredited nursing school
• Minimum of 6 months of Nurse Practitioner experience (preferred)

Youll be compensated per completed evaluation, with a competitive rate of $80$90 per visit; administrative time and mileage are also reimbursed.
