Step into an impactful, flexible per diem role dedicated to improving health outcomes in Allen County. As a Nurse Practitioner with home health experience, you will conduct comprehensive in-home risk assessments for Medicare Advantage plan members across Lima and its surrounding areas, including American Township and Shawnee Township. This position allows you to help patients understand their health betterwhile managing your own schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and evaluate medical history, current conditions, and medications
• Educate clients on disease management, medications, and healthy practices
• Identify gaps in care and suggest appropriate follow-up actions or resources
• Focus exclusively on assessment and educationno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner License in Ohio
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing program
• At least 6 months of current experience as a Nurse Practitioner (preferred)

Compensation & Perks:

• Earn $80-$90 per evaluation, with additional mileage reimbursement and compensation for administrative time
