Job Description

Take on an impactful per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you will conduct comprehensive evaluations for Medicare Advantage plan members across Cumberland County, including Hope Mills, Spring Lake, Eastover, and surrounding communities. Each visit typically lasts around 45 minutes and may include annual health assessments or post-discharge visits, allowing you to provide essential care and health education in patients homes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule
  • Commit to a minimum number of hours weekly or bi-weekly, as preferred

Key Responsibilities:

  • Complete in-home annual health risk assessments for Medicare Advantage members
  • Assess prior and current medical histories, evaluate medical conditions, and review medication regimens
  • Deliver patient education on medical conditions and medication management
  • Identify gaps in patient care and suggest appropriate recommendations
  • No prescribing of medication or development of treatment plans is required

Qualifications:

  • Active Nurse Practitioner license in the state of North Carolina
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
